Noninvasive mechanical ventilation for prevention of post-extubation respiratory failure.

چکیده
In their prospective multicenter randomized controlled trial, Su et al report that noninvasive mechanical ventilation (NIV) used in all patients after extubation did not reduce re-intubation rate, when compared to standard oxygen therapy.1 Nevertheless, we believe that the concept of “preventive” NIV requires not only early application of NIV but also careful selection of patients who are high risk for extubation failure and needing re-intubation. Some aspects of this study need mention, as these could have influenced the results:

a study published in the previous issue of Critical Care, Gonçalves and colleagues [1] observed a 33% reduction in re-intubation rate in the study group, due to eff ective management of bronchial secretions. Current evidence favors early implementation of noninvasive mechanical ventilation (NIV) in the post-extubation period to reduce re-intubation, especially in patients with hypercapnic respi...
متن کاملNoninvasive mechanical ventilation in immediate postoperative cardiac surgery patients.
BACKGROUND Noninvasive ventilation is routine in acute respiratory failure patients; nevertheless, the literature is controversial for its use in cardiac surgery postoperative period. OBJECTIVE To evaluate the effectiveness of preventive noninvasive ventilation in the immediate postoperative period of cardiac surgery, monitoring its impact until the sixth day of hospitalization. METHODS Thi...
متن کاملNoninvasive ventilation to shorten the duration of mechanical ventilation.
Noninvasive ventilation (NIV) successfully treats primary respiratory failure in chronic obstructive pulmonary disease (COPD), acute pulmonary edema, and, in some patients, hypoxemic respiratory failure. Increasingly clinicians have applied NIV in an effort to shorten the duration of mechanical ventilation by facilitating weaning and preventing or treating post-extubation respiratory failure. R...
متن کاملNoninvasive positive-pressure ventilation: a silver bullet for extubation failure?
Extubation failure, or the need for reintubation within 24–72 hours of extubation, occurs in up to 25% of critically ill patients. Extubation failure is associated with a markedly increased morbidity and mortality, including duration of mechanical ventilation, intensive care unit (ICU) and hospital stay, the need for post-acute-care hospitalization, and the need for tracheostomy.1,2 The most co...
متن کاملNoninvasive ventilation for prevention of post-extubation respiratory failure in obese patients.
Current recommendations for management of obese patients post-extubation are based on clinical experience and expert opinions. It was hypothesised that the application of noninvasive ventilation (NIV) during the first 48 h after extubation in severely obese patients would reduce post-extubation failure and avert the need for reintubation. Following protocol-driven weaning trials, 62 consecutive...
